📣 Why Is Open Enrollment Important?

Three Wire Systems’ Annual Open Enrollment (OE) for the 2025–2026 plan year begins Monday, October 13 and ends on Sunday, October 20. This is your once-a-year opportunity to:

Review benefit offerings
Add or remove eligible dependents
Enroll or make changes to your benefit elections

👉 Outside of Open Enrollment, changes are only allowed if you experience a Qualifying Life Event (such as marriage, birth/adoption, or loss of other coverage).

✅ Steps to Take Before October 20th

To ensure a smooth enrollment experience, please:

Log in to the Boon Group Online Enrollment System using the instructions provided
Review your Aetna plan options and benefit guides
Submit your elections — or your completed Hard Waiver Form with proof of other coverage
Enroll in the Opt-Out Ancillary Package if you are waiving medical
Confirm your mailing address is current to receive your new Aetna ID card

👤 For SCA Employees

Your current Cigna Medical, Dental, and Vision coverage will end October 31, 2025. Beginning November 1, 2025, benefits will be provided through Aetna and administered by The Boon Group.

You must actively select one of the following Aetna plan options:

Aetna PPO $1,500 Plan
Aetna PPO HSA $2,500 Plan
Aetna Fixed Indemnity Plan (default if no valid waiver submitted)
Opt-Out Ancillary Package (required if waiving medical with approved Hard Waiver)

📬 You will receive a new Aetna ID card if enrolled in medical.

🧾 Other Benefits for SCA Employees

Group Life & Disability: Remain with Unum – current coverage will roll over
Voluntary Benefits: Accident, Critical Illness, Hospital Indemnity – continue with Unum
FSA: Remains with ABG – re-election required each year
HSA: Available with PPO HSA Plan – employee-funded; election required

❗ Waiving Medical Coverage

To waive Aetna medical coverage, you must submit:

A completed Hard Waiver Form
Proof of other creditable group coverage, such as:
- A copy of your insurance ID card (with effective date), or
- A letter from your plan sponsor confirming active coverage and start date

🧾 If your waiver is approved, you must still enroll in the Opt-Out Ancillary Package, which includes:

$10,000 Term Life Insurance
Telehealth Services

🗓 Important Note: Open Enrollment for benefits effective December 1, 2025 will take place in early November. Be sure to check back for updates and key deadlines.

Enroll in Benefits

LOGGING IN | ADP

Log into your ADP website. Note: If this is your first time logging in, or you need help getting started, click the appropriate link for instructions and assistance.
Access the Employee Self Service Website.
Click "User Login".

Note: Information or activity notification pages may display. Please respond as needed to continue.

After you log in, use the Enrollments page to review your current benefits, if applicable, and make your Open Enrollment selections. To view your current enrollment details:

Start by going to Myself > Benefits > Enrollments.
Click the "Plan Name" to view details of the enrollment and to edit beneficiary assignments.
On the Enrollment Details Page, review enrollment information for the plan you selected. Note: For additional assistance, click the "Help" icon.
When you have completed your review, click "Done". You can select other plans to review as needed.

Tip: The Help icon and the Full Screen icon display at the top of the page (following the name of the page). Click the "Help" icon to find additional assistance and information. Click the "Full Screen" icon to expand the page you are viewing. Click the "Restore" icon to return to the normal view.

MAKING ENROLLMENT CHANGES

Start by going to Splash Page > Start This Enrollment OR Myself > Benefits > Enrollments.

If the Splash Page is activated by your HR admin and you qualify for Open Enrollment, during your Open

Enrollment period you will be prompted by the Splash Page upon login. To enter the Open Enrollment

process, click on "Start This Enrollment".

Hard Waiver Required

To waive enrollment in the Aetna medical plan, you must provide both a completed waiver form and acceptable proof of other coverage. Follow the steps below to ensure your waiver is submitted correctly and on time.

✅ Step 1: Gather Acceptable Proof of Other Coverage

You must have active group medical coverage under another plan to qualify. One of the following is required:

A copy of your current Insurance ID Card
- Must clearly display the policyholder’s name
- Must show a valid effective date

A letter from your other plan sponsor (HR Dept or Insurance Company)
- Must confirm active coverage
- Must include the start date of coverage
- Must identify the carrier and type of plan

❗ Accepted Coverage Types:

Spouse’s employer-sponsored group plan
TRICARE
VA Benefits

🚫 Not Accepted:

Medicare
Medicaid
Individual Marketplace / Exchange plans

✅ Step 2: Complete the Hard Waiver Form

Access the Hard Waiver Form via the Benefits Website or directly from your HR team.
Complete all required sections including:
- Your name and contact information
- Type of other coverage you are enrolled in
- Signature and date

✅ Step 3: Submit the Waiver and Proof of Coverage

Submit the following no later than October 20, 2025:

Your completed Hard Waiver Form
Your Proof of Other Coverage (ID card or confirmation letter)

✅ Step 4: Enroll in the Required Opt-Out Ancillary Package

If your Hard Waiver is approved, you must still elect the Opt-Out Ancillary Package, which includes:

$10,000 Term Life Insurance
Access to Telehealth Services

This is a required benefit for all employees waiving medical.

📬 Confirmation

Once your documents are reviewed and accepted, you will receive confirmation of your waiver status and enrollment in the Opt-Out Ancillary Package.

Enter your Zip Code
Select a Plan → Under AFIP/AFBP Plans → Select Open Choice PPO or Open Choice PPO - Indemnity Plans
Click Continue

Aetna Dental Provider Search:

Go to Find a Dental Provider
Select a Plan → Under Dental Plans → Select Dental PPO/PDN

Click Continue
